You got your lipid panel back. Next to your LDL cholesterol there’s no red flag — just the word “normal,” or a number sitting quietly inside the reference range. Your doctor says you’re fine. For many people, in that moment, the conversation ends.
But “normal” is answering a narrower question than most people assume. It tells you that your result looks like most other people’s results. It does not tell you whether your arteries are on the healthiest possible trajectory for the decades ahead. Those are two different questions — and the gap between them is where a lot of preventable cardiovascular risk hides.
Two rulers, two very different questions
A lab reference range is a statistical tool. It’s built by measuring a large group of people and drawing lines around where most of them fall. “Normal” simply means: your value looks like the crowd’s.
The catch is that the crowd isn’t especially healthy. Cardiovascular disease is the leading cause of death across most of the countries where this testing is common. So a range built around “most people” is a range built around a population in which heart disease is common. Being average, here, is not the same as being safe.
An optimal target asks a different question: what level is associated with the lowest long-term risk to your arteries — for someone with your risk profile? That target is usually lower than the top of the “normal” range, and it isn’t the same number for everyone.

Why the gap matters: your arteries keep score
Atherosclerosis — the gradual buildup that narrows arteries — isn’t a switch that flips in your sixties. It’s a slow accumulation that begins decades earlier. The particles that ferry cholesterol into the artery wall do a little damage every year they circulate at a given level. It’s the total exposure — how high, for how long — that builds plaque.
That’s the idea behind a phrase we use a lot at CardioIQ: your 80-year-old arteries are built at 40. A number that’s merely “acceptable” for decades still adds up. A number that’s genuinely low, held early and for the long run, adds up to far less. This is why prevention is measured in decades, not in single readings — and why “not flagged” is a lower bar than “optimized for the long game.”
What the guidelines actually target
Here’s something the single word “normal” hides: major cardiovascular guidelines don’t set one cholesterol goal for everyone. The European Society of Cardiology and European Atherosclerosis Society (ESC/EAS) set LDL cholesterol targets by risk category — and the higher your risk, the lower the target goes.
Simplified from ESC/EAS dyslipidaemia guidance. Your physician sets your risk category using your full profile, and the higher-risk targets also generally call for at least halving LDL from your starting point. These are guideline targets to discuss with your doctor — not a diagnosis.
Notice what this means. A single LDL value — say, one that a standard report calls “normal” — can be comfortably on-target for a low-risk person and clearly above target for someone at high risk. Same number, different meaning. “Normal” can’t see your risk. Optimal is defined by it.
Two things a “normal” cholesterol result can still miss
Even a reassuring LDL number can leave real risk unmeasured. Two markers are the usual culprits:
ApoB — the particle count
LDL cholesterol measures the cholesterol carried inside your LDL particles. But it’s the number of particles — not the cholesterol they carry — that drives plaque. In a substantial share of people, especially those with higher triglycerides, insulin resistance, or metabolic syndrome, these two disagree: cholesterol looks fine while particle count runs high. Lp(a) — the inherited factor

What changes your optimal
There’s no single optimal number that fits everyone, because your target moves with your risk. The factors that push it lower include: a family history of early heart disease; high Lp(a); insulin resistance or metabolic syndrome (often signalled by high triglycerides, low HDL, or elevated fasting insulin); high blood pressure, diabetes, or kidney disease; smoking; and simply the years of exposure you’ve already accumulated.
The more of these you carry, the further “normal” and “optimal” pull apart — and the more a one-size population range undersells your real target. It’s also why no single value should be read in isolation: a number only means something once it’s placed against your whole picture.

What to actually do with this
None of this is a reason to panic, and none of it is a prescription. It’s a reason to ask a sharper question than “is it normal?”
- Know your real targets. Ask your physician which LDL — and ideally ApoB — target fits your risk category, not just whether you’re “in range.”
- Measure the right things once. If you’ve never had ApoB or Lp(a) checked, that’s a reasonable conversation to have — Lp(a) especially, since once in a lifetime is usually enough.
- Start with lifestyle. Nutrition quality, regular activity, sleep, and not smoking move the whole panel and are always first-line.
- If targets aren’t met, discuss options. Several classes of therapy lower atherogenic particles effectively; which one (if any) fits you is a decision for you and your physician, based on your risk — not something to self-prescribe.
Earlier and steadier beats later and dramatic. The advantage of asking “is it optimal?” in your forties or fifties is that small, sustained changes have decades to compound.
