The result is a lower circulating ApoB and LDL-C — fewer of the particles that drive atherosclerosis.

This is why statins are described as treating the cause of atherosclerotic risk rather than a symptom: they reduce the particle burden that physically builds arterial plaque over time.

Intensity, not just "on or off"

The specific agent and dose are matched to the person's risk, targets, and tolerability by their physician; there is no single "right" statin or dose that applies to everyone.

What the evidence shows

Statins are supported by one of the largest bodies of randomized evidence in all of medicine. Pooled analyses of many trials — most notably by the Cholesterol Treatment Trialists' (CTT) Collaboration — show a consistent finding: lowering LDL-C with a statin reduces the risk of major cardiovascular events, and the benefit is proportional to how much the particle burden falls and how long it stays low. The relationship holds across a wide range of starting risk levels.

This is the same principle that runs through the rest of cardiovascular prevention: it's the cumulative exposure to atherogenic particles that drives disease, so reducing that exposure — earlier and for longer — is what changes outcomes.

Side effects: an honest, calm picture

Concern about side effects is the single most common reason people hesitate, so it deserves a straight answer rather than either dismissal or alarm.

Muscle symptoms

Muscle aches are the most talked-about statin concern. The reality from blinded trials is reassuring: the large majority of people tolerate statins well, and in carefully designed studies, most muscle symptoms occurred just as often on placebo as on the active drug — a pattern known as the nocebo effect. Genuine statin-associated muscle symptoms do occur in a minority, and they are almost always manageable — by adjusting the dose, switching agents, or other strategies your physician can offer. Serious muscle injury is rare.

Blood sugar and other effects

Statins are associated with a small increase in the risk of developing type 2 diabetes, mostly in people already close to that threshold. Importantly, across the evidence, the cardiovascular benefit for those who need a statin outweighs this small risk. Effects on the liver are usually minor; your physician monitors what needs monitoring.

Important: if you're already taking a statin and experience symptoms, the safest step is to speak with your physician before changing anything — not to stop on your own. Stopping a needed medication carries its own risk, and most side-effect problems have a solution that keeps you protected.

Statins and Lp(a): clearing up a common worry

You may read that statins can slightly raise Lp(a). This is true — but it is not a reason to avoid statins. The effect on Lp(a) is small, while the reduction in ApoB and overall cardiovascular risk is large and well-proven. For someone with high Lp(a), controlling every other risk factor — which is exactly what a statin does by lowering ApoB — is a cornerstone of management, precisely because Lp(a) itself can't yet be lowered with an approved drug.

If a statin isn't enough — or isn't tolerated

When a statin alone doesn't bring ApoB and LDL-C to target, or when someone genuinely can't tolerate one, other options exist. Additional classes of lipid-lowering therapy — such as ezetimibe, PCSK9 inhibitors, and bempedoic acid — can be added or substituted.
